Certilytics, Inc. has introduced an AI-powered model that predicts a patient’s Social Determinants of Health (SDOH) risk, enabling interventions at both the population and member levels to improve healthcare outcomes with cost and efficiency measures.

Health plans, employers, and healthcare providers can use Certilytics’ Patient SDOH Index (PSI) Score to develop new strategies for addressing social health obstacles, such as a lack of transportation, food, or access to non-emergency medical care.

The model is not only capable of predicting each patient’s SDOH risk, but it also applies deep learning to create individual patient “personas” that identify the top factors that increase that risk for personalized intervention strategies.
